What should a referral include?

• Patient name, date of birth, and best contact number
• Reason for referral and relevant history
• Current medications
• Recent drug screen results, if available
• Referring provider or agency contact information

Who is a good fit?

Adults in Tennessee with opioid use disorder — prescription opioids, heroin, or fentanyl — who can participate in video visits and periodic drug screening. Patients who also need psychiatric care or therapy can be connected to our parent practice, Shrinkty Behavioral Health.
